Five times Sam hit Jack.
1. P3X-595. She didn't hit him when he broke up the Sam/Teal'c snogfest, but she'd had quite enough of his meddling when he interrupted her and Daniel.
2. Casino Night at the SGC where Sam was a blackjack dealer. Jack kept saying "hit me" to her chest and so she slugged him.
3. When Jack called Prime Minister Bobbiezze "boobies" during a conference, Sam proceeded to kick him under the table. And kept kicking him when he wouldn't stop, each kick getting harder. It was only after the fifth kick that she realized she had been kicking Teal'c, whose stoicness let him display only a slight twitch in his left eye. She angled her foot the sixth time and nailed Jack, which resulted in a string of expletives. However, he stopped saying "boobies."
4. When Jack proposed at a hockey game by having the team mascot, a giant hamster, give her the ring. If that wasn't bad enough, the hamster then proceeded to place a beer hat on her, compliments of the franchise. And the whole thing made the 5 o'clock news.
5. After Jack admitted that there really were fish in his pond all along, and he was just messing with her so she'd go nuts in contemplating time paradoxes.
